The Medical Home—also known as Patient-Centered Medical Home or PCMH is a primary healthcare delivery model that works to centralize healthcare for you and your family. Patient treatment is coordinated through a primary care physician, facilitating partnerships between patients, doctors, and their families. Whole-Person Approach: The personal primary physician provides all the patient's health care needs and arranges necessary care with other medical specialists.

Physician Directed Medical Practice: The personal primary physician manages and directs a medical team in the practice who collectively are responsible for the ongoing care of the patients.

Care is coordinated and/or integrated: Care is coordinated within the complex healthcare systems of hospitals, specialists, nursing homes, and home health agencies, as well as the patient’s family and any community-based or social services. The goal is to ensure all necessary resources are utilized and accessible--being a community partner is key!

They accept most insurances, all KY Medicaid plans, OH Medicaid (CareSource & Molina), Medicare and offer a sliding fee scale for patients without insurance or who are underinsured (high deductible plans, etc). Discounts provided by PrimaryPlus' sliding fee scale are up to 75% based on application approval. Plus, PrimaryPlus offers on-site pharmacy services that offer significant savings to their patients through their 340B program.
